RESUMO

Objective:To investigate the effect of right stellate ganglion block (RSGB)-serratus anterior plane block (SAPB) combined with general anesthesia in thoracoscopic radical resection of lung cancer.Methods:A total of 90 patients who planned to undergo thoracoscopic radical resection of lung cancer in Xiangya Changde Hospital from March 2020 to September 2021 were prospectively selected and divided into 3 groups by random number table method: general anesthesia group (G group), (SAPB)+ general anesthesia group (SG group), RSGB+ SAPB+ general anesthesia group (RSG group), 30 cases in each group. The SG group received SAPB on the operative side before general anesthesia, and the RSG group received RSGB+ SAPB on the operative side before general anesthesia. After the blocking effect was determined, all patients were given general anesthesia in the same scheme according to their weight, and patients were given patient-controlled intravenous analgesia (PCIA) after surgery. The mean arterial pressure (MAP) and heart rate (HR) were recorded before anesthesia (T 0), before intubation (T 1), 1 min after intubation (T 2), 5 min after intubation (T 3), at extubation (T 4) and 5 min after extubation (T 5). The intraoperative dosage of remifentanil, incidence of nausea and vomiting (PONV) within 24 hours after surgery, number of additional PCIA within 24 hours, the Visual Analogue Scale (VAS) of static and dynamic pain, the Bruggrmann Comfort Scale (BCS) and Richard Campbell Sleep Questionnaire (RSCQ) 24 hours after surgery were recorded. Results:Compared with T 0, the MAP and HR in 3 groups were increased 1 min after intubation (T 2) and at extubation (T 4), but the increases in RSG group were significantly less than those in G and SG groups (all P<0.05). The remifentanil dosage, PONV incidence and PCIA supplemental times in SG and RSG groups were less than those in G group, and the BCS score and RSCQ score were higher than those in G group (all P<0.05); the BCS score and RSCQ score in RSG group were higher than those in SG group (all P<0.05). Conclusions:RSGB+ SAPB combined with general anesthesia in thoracoscopic radical resection of lung cancer has little circulation fluctuation, good postoperative analgesia effect, less adverse reactions and high comfort level.

OBJECTIVES@#There are clinical reports of nerve injury caused by ropivacaine. The mechanism for nerve injury induced by ropivacaine has not been fully clarified. This study aims to investigate the changes of pain threshold and L3 spinal cord genomics at 6 h and 24 h after intrathecal injection of 0.5% and 1.0% ropivacaine, and to explore the underlying mechanisms for nerve injury caused by ropivacaine.@*METHODS@#A total of 30 male Sprague Dawley rats weighing 220-260 g were successfully implanted with microspinal catheter. The rats were randomly divided into 5 groups (each n=6): a control group (given saline), a ropivacaine group 1 and a ropivacaine group 2 (both given 1% ropivacaine), a ropivacaine group 3 and a ropivacaine group 4 (both given 0.5% ropivacaine). The rats received continuous intrathecal injection of corresponding drugs at 8.3 μL/h for 24 h via an implanted intrathecal catheter followed by 24 h-pause of injection for the ropivacaine group 2, the ropivacaine group 4 and the control group, 6 h-pause of injection for the ropivacaine group 1 and the ropivacaine group 3. For each group, the observation of behavioral change and the paw withdrawal mechanical threshold (PWMT) was conducted immediately after the injection and again after the pause of injection. After the PWMT observation, the rats were dissected to acquire L3 spinal cords. Illumina sequencing was applied to construct gene libraries. Then the statistical methods were used to find out differentially expressed genes between the groups. Gene Ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathway analysis were conducted for those genes. Real-time RT-PCR was used to determine different expressions of some of those genes.@*RESULTS@#Compared with control group, the PWMT got higher in the ropivacaine group 1-4 and was positively correlated with concentration, negatively correlated with discontinuation duration. Compared with control group, the ropivacaine group 1 had 488 differentially expressed genes, of which 456 were up-regulated and 32 were down-regulated; the ropivacaine group 2 had 1 194 differentially expressed genes, of which 1 092 were up-regulated and 102 were down-regulated; the ropivacaine group 3 had 518 differentially expressed genes, of which 384 were up-regulated and 134 were down-regulated; and the ropivacaine group 4 had 68 differentially expressed genes, of which 46 were up-regulated and 22 were down-regulated. GO enrichment analysis and KEGG signaling pathway analysis showed that most of these differentially expressed genes were related to signaling pathways of inflammatory response.@*CONCLUSIONS@#After intrathecal injection of 0.5% ropivacaine and 1.0% ropivacaine for 24 h, the differentially expressed genes in L3 spinal cord of rats are mainly related to signaling pathways of inflammatory response.

Objective To investigate the epidemiological characteristics and main risk factors of chronic kidney disease in the elderly in Tianmen, Hubei Province. Methods A descriptive method was adopted in this study. Using stratified sampling, a total of 720 local residents over 65 years old in Tianmen of Hubei Province were selected from January 2017 to January 2020 as the research subjects. The general information of patients with chronic kidney disease was compared, and the difference in the characteristics of patients with different disease progression was compared. The risk factors for chronic kidney disease and disease progression were analyzed. Results Through the diagnosis of chronic kidney disease, a total of 82 patients with chronic kidney disease were found, including 33 patients in stage I, 26 patients in stage II and 23 patients in stage III. As the disease progressed, the patient's age, serum uric acid, triglyceride, total cholesterol, fasting blood glucose, serum creatinine, and the incidence of chronic diseases all increased significantly. The results of correlation analysis showed that the incidence of chronic kidney disease and the progression of the disease were positively correlated with age, body mass index, serum uric acid, triglyceride, total cholesterol, fasting blood glucose, serum creatinine level, diabetes, hypertension, and dyslipidemia, which were independent risk factors for chronic kidney disease. Conclusion The prevalence of chronic kidney disease in Tianmen was 11.39%. Higher age, serum uric acid, blood lipid, blood pressure and blood glucose levels are all important factors for the progression of chronic kidney disease.

To investigate the effect of SB203580, a p38MAPK specific inhibitor, on ropivacaine-induced cytotoxicity in PC12 cells.
 Methods: PC12 cells were divided into three groups: the normal group (Group N), cells were cultured for 48 h; the ropivacaine group (Group R), cells were cultured with 15 mmol/L ropivacaine hydrochloride for 48 h; the ropivacaine+SB203580 group (Group R+S), cells were cultured with 15 mmol/L ropivacaine hydrochloride plus 10 μmol/L SB203580 for 48 h. The cell survival rates were detected by MTT assay. The protein levels of cleaved caspase-3, phosphor-p38 (p-p38) and cystolic cytochrome C (Cyt C) were detected by Western blotting.
 Results: Compared with the Group N, the number and survival rate of PC12 cells in the Group R and the Group R+S were significantly reduced (all P<0.05); the number and survival rate of PC12 cells in the Group R+S were significantly higher than those in the Group R (both P<0.05). Compared with the Group N, the levels of p-p38 and cleaved caspase-3, and the content of cytoplasmic Cyt C in the PC12 cells from the Group R and the Group R+S were significantly enhanced (all P<0.05); compared with the Group R, the levels of p-p38 and cleaved caspase-3, and the content of cytoplasmic Cyt C in the PC12 cells from the Group R+S were decreased (all P<0.05).
 Conclusion: The ropivacaine-induced cytotoxicity can be attenuated via inhibition of p38MAPK; which is related to decrease in Cyt C content and cleaved caspase-3 expression.

Objective To investigate the appropriate ratio of propofol and remifentanil in total in travenous anesthesia for adult fiberoptic bronchoscopy.Methods 122 cases of adult patients who were examined under total intravenous anesthesia (TIVA) with fiberoptic bronchoscopy in Xiangya hospital were randomly divided into three groups regarding the mixed proportion of propofol and remifentanil:group Ⅰ including 42 cases,group Ⅱ 41 cases,group Ⅲ 39 cases;the mixed proportion of propofol and remifentanil in group Ⅰ was 400 mg∶ 1 mg,in group Ⅱ was 1 000 mg∶ 1 mg,and in group Ⅲ was 2 000 mg∶ 1 mg,were given during TIVA.In the surgery,the bispectral index (BIS) values were sustained between 45 and 60 and we compared the changes of vital signs,airway resistance,the incidence of bucking and airway spasm before and after anesthesia.Results Compared with group Ⅱ,the airway resistance and the incidence of bucking and airway spasm in group Ⅰ both significantly increased and the blood pressure fluctuation in group Ⅲ was greater significantly (P < 0.05).Conclusions The appropriate ratio of propofol and remifentanil is 1 000 mg∶ 1 mg with total intravenous anesthesia in fiberoptic bronchoscopy,which results a stable hemodynamics,lower airway resistance,and lower incidence of bucking and airway spasm.

Objective To assess the value of brain atrophy and lesion index(BALI),based on magnetic resonance imaging(M RI),in the evaluation of brain aging. Methods 169 healthy older adults were divided into five age groups(40-49,50-59,60-69,70-79,and 80-89 years).MRI scans were evaluated by the 3.0T GE signa and the BALI rating schemes,based on the T1 weighted(T1WI),T2 weighted(T2WI),and T2 weighted fluid attenuated inversion recovery(T2-FLAIR),and T2*weighted gradient-recalled echo(T2 * GRE)images were recorded. Results Based on T1WI,T2WI,T2-FLAIR and T2 * GRE,total scores of BALI increased with age,and showed significant differences between the five age groups(F= 35.35,42.87,46.57,and 54.15,respectively;all P= 0.000).In addition,BALI scores from each sequence were correlated with age(T1WI:r=0.71;T2WI:r=0.73;T2-FLAIR:r=0.73;T2*GRE:r=0.77;all P<0.01).Furthermore,T2*GRE was most sensitive to microbleeds and T2-FLAIR revealed a greater level of deep white matter(χ2 =53.47,P= 0.000)and periventricular lesions(χ2 =29.93,P=0.000)than other sequences. Conclusions The T1WI,T2WI, T2-FLAIR and T2 * GRE,BALI scores can be used to assess whole brain structural changes with aging and provide semi-quantitative indicators for the assessment of brain health.

Objective To investigate the effects of low carbohydrate on body weight,glucose and lipid metabolism,sex hormones and conception of polycystic ovary syndrome (PCOS) with obese patients.Methods Sixty-four patients with PCOS and obesity and fertility were enrolled in this study from November 2015 to December 2016,and were randomly divided into low-carbohydrate diet group (study group) and control group.Before and after the intervention,the body weight,body fat composition analysis,reproductive endocrine hormone,glucose and lipid metabolism,menstrual cycle changes and conception were measured at 12 weeks after intervention.Results (1) After 12 weeks of treatment,the body weight,waist circumference,Body Mass Index(BMI)and visceral fat area of the study group were lower than those of the control group,the difference was statistically significant (P < 0.05).(2) After 12 weeks of treatment,fasting blood glucose (FPG) of the two groups had no significant difference (P > 0.05),while the 2 h postprandial blood glucose (PPG) of study group was significantly lower than the control group,the difference between the two groups was statistically significant (P < 0.05).After treatment,the fasting insulin (FINS) of the study group and control groupwas respectively decreased by 48.8% and 25.3% compared with the baseline,the difference was statistically significant (P < 0.05).(3) After treatment,the testosterone (T),free androgen index (FAI) and luteinizing hormone (LH) of the two groups decreased,compared to the control group,the study group decreased significantly (P < 0.05).(4) The recovery rate of menstrual cycle and natural pregnancy rate in the study group were significantly better than those in the control group (P < 0.05).Conclusions Low carbohydrate diet combined with metformin and moderate intensity exercise can effectively reduce weight,improve glucose,lipid metabolism and sex hormone levels,improve the natural pregnancy rate.

OBJECTIVE:To investigate the effectiveness and economics of 10 mg/d rosuvastatin and 20 mg/d atorvastatin in the treatment of hyperlipidemia (HLP). METHODS:The information of 180 HLP patients selected from Tianmen Municipal First People's Hospital during Mar. 2015-Feb. 2016 were divided into group A and B according to medication regimen,with 90 cases in each group. Group A was given Atorvastatin calcium tablet 20 mg,qd;group B was given Rosuvastatin calcium tablet 10 mg,qd. Treatment course of 2 groups lasted for 8 weeks. Blood lipid indexes before and after treatment,lipid-lowering efficacy,the rate of qualified blood lipid and the occurrence of ADR after treatment were compared between 2 groups. Cost-effectiveness analysis was adopted for economic evaluation. RESULTS:Before treatment,there was no statistical significance in the levels of blood lipid in-dexes between 2 groups (P>0.05). After treatment,TC and LDL-C levels of 2 groups were significantly lower than before treat-ment,and those of group B were significantly lower than those of group A,with statistical significance(P0.05). The costs of group A and B were 488.32,436.24 yuan,and cost-effectiveness ratios were 5.63,4.46;incremental cost-effectiveness ratio was -4.69. The plan of group B had cost-effective-ness advantage. The results of cost-effectiveness analysis were supported by sensitivity analysis. CONCLUSIONS:In the view of short-term efficacy,10 mg/d rosuvastatin plan is better than 20 mg/d atorvastatin plan in lowering lipid and has cost-effectiveness advantage,and both have similar safety.

Objective To evaluate the effect of neurotrophin-3 (NT-3) on the expression of serine/threonine protein kinase (Akt) during ropivacaine-induced neurotoxicity to the spinal cord of rats.Methods Healthy adult male Sprague-Dawley rats,aged 1-2 months,weighing 280-320 g,were used in the study.A catheter was inserted at L5,6 interspace into the epidural space of rats.A total of 108 rats,in which intrathecal catheters were successfully implanted,were randomly divided into 3 groups (n =36each):control group (group C),1% ropivacaine group (group R),and 1% ropivacaine + NT-3 group (group NT).The equal volume of normal saline was given in group C,1% ropivacaine 0.12 ml/kg was injected via the intrathecal catheter once every 1.5 h for 8 times in total in R and NT groups.In addition,NT-3 0.1 mg/kg was simultanenously injected via the intrathecal catheter in group NT.On days 1,3,5,7,14 and 28 after the end of administration (T1-6),6 rats were sacrificed in each group.Their lumbar enlargements were removed for determination of neuronal apoptosis (using TUNEL) and Akt expression (by immuno-histochemistry).The apoptotic rate was calculated.Results Compared with group C,the apoptotic rate was significantly increased at T1-4,and Akt expression was significantly up-regulated at T1-3 in group R,and the apoptotic rate were significantly increased,and Akt expression was significantly up-regulated at T1-3 in group NT (P<0.05).Compared with group R,the apoptotic rate was significantly decreased at T3,4,and Akt expression was significantly down-regulated at T2.3 in group NT (P<0.05).Conclusion The mechanism by which NT-3 reduces ropivacaine-induced neurotoxicity to the spinal cord may be related to down-regulation of the expression of Akt in rats.

Objective To explore the correlation of urinary albumin/creatinine ratio (urinary albumin-to-creatinine ratio,UACR) and blood pressure in young people.Methods Totally 200 cases of hypertensive patients (aged 18 ~50 years old) from January 2012 to March 2014 and 200 healthy volunteers who had medical examination in our hospital were selected with a random sampling method for staff questionnaires,biochemical tests,and physical examinations.Biochemical tests included kidney function,blood glucose,blood lipids,blood uric acid,and UACR.First,400 cases were divided into four groups (1,2,3,and 4 groups) according to UACR quartiles from low to high.Second,400 cases were divided into three groups according to their blood pressure:normal blood pressure,prehypertension,and hypertension.The correlation of UACR and different groups of blood pressure was analyzed.Results For norm blood pressure group,prehypertension,hypertension,the corresponding UACR level (median:25% ~ 75% quartile) was 0.8 (0.6 ~ 1.2) mg/mmol,0.9 (0.7 ~ 1.4) mg/mmol,1.0 (0.8 ~ 1.7) mg/mmol;positive UACR rate was 0,2.6%,and 34.7%,three groups UACR levels and positive UACR rate index differences were statistically significant (P <0.05).After age factor was adjusted for 400 cases,both systolic and diastolic blood pressures were positively correlated with UACR levels.After body mass index,age,total cholesterol,and low-density lipoprotein cholesterol were adjusted for 400 cases,compared to group 1,the ratio of prehypertension to hypertension (95% confidence intervals) in groups 2,3,and 4 was 0.98 (0.80 ~ 1.20),1.29 (1.05 ~ 1.58),and 1.86 1.52 ~2.28).Conclusions Urinary albumin/creatinine ratio was positively correlated with prehypertension and hypertension in young people.

OBJECTIVE@#To investigate the effect of pretreatment with neurotrophin-3 (NT-3) on intrathecal ropivacaine in rats.@*METHODS@#A total of 144 male Sprague Dawley rats weighing 280-320 g were successfully implanted with microspinal cather following the improved methods of Yaksh. The rats were randomly divided into 4 groups and given saline (Group NS, n=36), 0.5% ropivacaine (Group M, n=36), 1% ropivacaine (Group R, n=36), and ropivacaine+NT-3 (Group T, n=36). The rats received 0.12 mL/ kg body weight of ropivacaine at 0.5% or 1%, or normal saline only, via an implanted intrathecal catheter at 90-min interval for 12 h in Group NS, M, R and T. In the meantime the rats also received NT-3 0.1 mg/kg in group T. On days 1, 3, 5, 7, 14 and 28, we assessed the paw withdrawal mechanical threshold (PWMT) and paw withdrawal thermal latency (PWTL), behavioural change and histopathological damage score changed for possible neuronal injury within the spinal cord.@*RESULTS@#Compared with Group NS and Group M, the PWMT and PWTL were significantly higher on 1, 3, 5 d and the histopathological damage score was significantly higher on 1, 3, 5, 7, 14 d in Group R (P<0.05). Compared with Group T, the PWMT and PWTL in Group R were significantly higher on 1, 3, 5 d and histopathological damage score was significantly higher on 5, 7, 14 d (P<0.05).@*CONCLUSION@#NT-3 pretreatment in mice has obvious protective effect against repeated intrathecal injection of 1% ropivacaine in the spinal nerve.

RESUMO

Potential target proteins binding to VirB4 of type Ⅳ secretion system were screened during Brucella infected bovine embryonic trophoblast cells .Brucella VirB4 genes were amplified by PCR with species-specific primers .Expression vector pGBKT7-virB4 was constructed and analysed by sequencing and restriction enzymes ,transforming to the yeast strain Y187 and testing self-activation and toxicity .The cells model and cDNA library of bovine embryonic trophoblast cells infected with Brucella abortus strain were constructed respectively .Utilizing yeast two-hybrid system was employed to screen the target proteins of bovine embryo trophoblastic cells which was conjunctive with virB4 .These proteins were detected by real-time fluo-rescence quantitative PCR .The results suggested that bait plasmid pGBKT7-virB4 was successfully transformed into the Y187 and there was no toxicity and self-activation;the cDNA library of bovine embryonic trophoblast cells infected with Brucella abortus strain was constructed .There screened 13 positive plasmids in which Q10 and SLC3A2 were up-regulated at the mRNA level .In this paper ,we reported the interactions between the VirB4 protein of Brucella and the bovine embryo trophoblastic cells ,which provide an upstream work for further elucidating the pathogenesis of Brucella infection of the host cell .

Objective To evaluate the role of oxidative stress in the spinal neurotoxicity induced by ropivacaine in rats.Methods Ninety pathogen-free male Sprague-Dawley rats,weighing 250-320 g,aged 3 months,in which intrathecal catheter was successfully implanted into L5,6 interspace without complications,were randomly divided into 3 groups (n =30 each) using a random number table:control group (group C),ropivacaine group (group R),and antioxidant Tempol group (group T).The rats received 1% ropivacaine 1.2 μg/g for 8 times at 1.5 h intervals through the catheter in R and T groups,while the rats received the equal volume of normal saline instead in group C.In T group,Tempol 20 μg/g was injected intrathecally at 4,8,12,24,48 and 72 h after the last injection of ropivacaine.The rats were sacrificed at 1,3,5,7 and 14 days after the end of ropivacaine injection,and their lumbar enlargements were removed for TUNEL staining to detect the cell apoptosis.SOD activity was determined by colorimetry and MDA content was measured using TBA photoelectric colorimetry.Apoptosis index was calculated.Results Compared with group C,apoptosis index and MDA content were significantly increased,and SOD activity was decreased in R and T groups.Compared with group R,apoptosis index and MDA content were significantly decreased,and SOD activity was increased in group T.Conclusion Oxidative stress is involved in the development of spinal neurotoxicity induced by ropivacaine in rats.

Objective To investigate metabolic characteristics and insulin resistance in newly diagnosed type 2 diabetes pa-tients with nonalcoholic fatty liver disease .Methods Two hundred patients with newly diagnosed type 2 diabetes mellitus including 140 with nonalcoholic fatty liver disease (NAFLD) (NAFLD group) and 60 without NAFLD (non-NAFLD group) were recruited. Metabolic characteristic were measured and recorded .homeostasis model assessment insulin resistance ( HOMA-IR) index and HOMA-C were used to assess insulin resistance .Results were compared between two groups .Results Compared with non-NAFLD group, body mass index (BMI), fasting blood glucose (FBG),alanine aminotransferase (ALT),gamma-glutamyl transferase (GGT), uric acid ( UA) , triglyceride ( TG) , low density lipoprotein cholesterol ( LDL-C) ,fasting insulin ( FINS) , fasting C peptide ( FCP) , gly-cated hemoglobin a1c (HbA1c), HOMA-IR, and HOMA-C were significantly higher in the NAFLD group ( P <0.05).Logistics re-gression analysis showed that BMI , TG, GGT, and UA were risk factots for NAFLD ( P <0.05, OR =1.82, 1.53, 1.37, and 1.09 ) .Conclusions Compared patients without NAFLD , newly diagnosed type 2 diabetes patients with NAFLD present more meta-bolic disturbance, and severely insulin resistance.Obesity and the increased level of postprandial UA , triglyceridemia, and GGT might increase the risk of NAFLD in the patients with newly diagnosed type 2 diabetes mellitus .

RESUMO

Objective To investigate the evolution of monkey brain isehemic penumbra(IP)area.Methods Seyen monkeys were used to make middle cerebral artery occlusion(MCAO)model by interventional methods.CT perfnsion imaging,MR diffusion weighted-imaging (DWI),perfusion weighted imaging(PWI)and T2W1 were performed at 1,5,10;15,20 and 24 h after MCAO respectively.Four regions of interest of infarct lesion were measured.Point 1 was at the infarct core.point 3 was at the infamt margin,and point 2 was at the midpoint between point 1 and 3.Point 4 demonstrated normal signal intensity adjacent to high signal intensity.Parameters measured included cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), apparent diffusion coefficient (ADC) and negative enhancement integral (NEI).The relative ratios between infarct lesions and the corresponding contralateral normal brain were calculated(rCBF, rCBV, rMTY, rADC and rNEI).The IP areas were calculated by two methods: IP thresholds combined with self-made computer software, and PWI( MrlT)-DWI mismatch.ANOVA and ROC analysis were used. Results Five of 7 monkey MCAO models were made successfuUy. There were signitlcanfly difference of rCBF and rNEI within 20 h, of rCBV within 15 h, of rADC within 10 h, of rMTT at24 h (P<0.05).ROI 1,2 and 3 values as following: rCBF: 1 h(0.160 ±0.034, 0.310 ±0.037,0.540±0.107), 5 h(0.098±0.029, 0.157 ± 0.052, 0.427 ±0.116), 10 h(0.072 ±0.023, 0.097 ±0.028, 0.209 ± 0.070), 15 h(0.054 ± 0.017, 0.069 ± 0.015, 0.166 ± 0.049), 20 h(0.038 ± 0.011,0.026± 0.007, 0.092±0.013); rNEI: 1 h(0.219 ± 0.085, 0.303 ± 0.099, 0.463 ± 0.132), 5 h (0.143±0.057, 0.195± 0.055, 0.348±0.127), 10 h(0.127 ± 0.029, 0.171 ± 0.058, 0.259 ±0.079), 15 h(0.128 ±0.024, 0.164 ±0.031, 0.217 ±0.030), 20 h(0.075±0.019, 0.147±0.058,0.129 ±0.045) ; rCBV: 1 h(0.594 ± 0.199, 0.804 ± 0.099, 1.021 ±0.169), 5 h(0.457±0.103,0.462±0.145, 0.815±0.201), 10 h(0.222 ±0.046, 0.249±0.065, 0.529 ±0.135), 15 h(0.201 ±0.047, 0.187 ±0.055, 0.361 ±0.083) ; rADC: 1 h(0.515 ±0.115, 0.667±0.097, 0.761 ±0.106),5 h(0.488 ±0.100, 0.539 ±0.107, 0.674 ±0.099), 10 h(0.456 ±0.057, 0.549 ±0.049, 0.590 ±0.081 ) ; 24 h rMTT(4.163 ± 1.179, 4.192± 1.607, 2.397±0.909).The thresholds of IP were 0.203 of rCBF, 0.483 of rCBV, 0.571 of rADC and 0.250 of rNEI respectively.The values measured using the method of IP thresholds combined with software were larger than PWI(MTr)-DWI mismatch region before 15 hours, but were smaller at 20 h and 24 h. The area of IP was 20%-38% of infarct area at 1 h,15%-35% at 5-10 h, 13%-25% at 15 h, 9%-15% at 20 h, and 3%-12% at 24 h.Conclusion The time window of IP in monkey MCAO model was 15%-20 hours.At the early phase of infarction, IP was present within the region of high signal intensity on DWI.PWI-DWI mismatch method could not estimate IP areas accurately.Areas evaluated with CT perfusion (MrIT) and DWI mismatch were much closer to the actual IP areas.

OBJECTIVE@#To investigate the behavioral and ultrastructural changes of intrathecal administration of different concentrations of ropivacaine for 12 h.@*METHODS@#Thirty male SD rats were randomly divided into 5 groups (6 rats in each group):group N (control), group A (ropivacaine 0.25%), group B (ropivacaine 0.5%), group C (ropivacaine 0.75%),and group D (ropivacaine 1.0%). A polyurethane microcatheter was inserted into the lumbar subarachnoid space 8 cm according to Yakshos intrathecal administration. The rats in group N received saline 0.12 mL/kg for 8 times at 1.5 h interval through the catheter, and the rats in the other groups received different concentrations of ropivacaine in the same way as in group N. The poster paw withdrawal latency to heat (PWHL) and mechanical stimulation (von Fray filament) (PWML) were measured the day before the intrathecal administration and 12 hours after the first intrathecal administration of ropivacaine. Motor function (MF) was measured after the last intrathecal administration. After the behavior test, the rats were sacrificed and the lumber segments of the spinal cord were immediately removed for electron microscopic examination.@*RESULTS@#A total hind limb paralysis was seen at 30 seconds and intramuscular strain gradually came back 10~60 minutes after the intrathecal administration of ropivacaine in group A, B, C, and D, but not in group N. The recovery time of motor block of group A was the shortest (P<0.05), that of group D was the longest,and that of group B and C was between group A and D. Intrathecal administration of different concentrations of ropivacaine did not affect the percent maximum possible effect (%MPE) of PWHL and PWML. Electron microscopic examination showed that the spinal cords were normal in group N and A, slight edema of mitochondria and endoplasmic reticulum (ER) in group B, loosened fibrous layers in medullary sheath, edema and local degeneration of neuraxis in group C,and shrinkage of nuclear membrane, serious edema of ER, vacuolus change of mitochondria and local demyelination in group D.@*CONCLUSION@#Ropivacaine (0.5%, 0.75%, and 1.0%) administered intrathecally for 12 hours causes different degrees of ultrastructural changes in the spinal cord depending on concentrations.

Objective To discuss the MRI characteristics of pituitary abscess,so that to improve the diagnosis of it.Methods MR imaging data in 8 cases with pituitary abscess proved by operation and pathology,and antibiosis therapy were retrospectively analyzed in combination with review of literatures.Of 8 cases,3 were male and 5 were female,the mean age was 53.3 years(ranged 36~62 years).Results MRI findings included hypophysis enlargement,isointensity in 6 cases and slight hyperintensity in 2 cases on T1WI;isointensity in 4,hyperintensity in 2 and mixed iso-and hyperintensity in 2 on T2WI;hyperintensity in one on DWI.On contrast-enhanced scan,all pituitary masses showed ring enchancement,6 cases showed thickened walls which were slightly lamellar;7 cases demonstrated pituitary stalk thickening and enchancement.Conclusion Pituitary abscesses are of certain typical MRI characteristics.

<p><b>BACKGROUND</b>Chemotherapy is very important in the treatment of advanced non-small cell lung cancer (NSCLC). The aim of this study is to evaluate the efficacy, clinical benefit and toxicity of combined chemotherapy with domestic gemcitabine (ZEFEI) plus carboplatin in the treatment of advanced NSCLC.</p><p><b>METHODS</b>Thirty-four previously untreated patients with advanced NSCLC (stage III-IV) received domestic gemcitabine of 1000mg/m² on days 1, 8, and carboplatin of AUC 5 on day 1, with 21 days as a cycle. Each patient received at least three cycles.</p><p><b>RESULTS</b>The total clinical response rate (complete and partial response) was 44% (15/34). Overall clinical benefit rate was 53% (18/34). The main toxicities were hematological toxicities. The rate of grade III-IV leukopenia and thrombocytopenia was 47% and 24% respectively.</p><p><b>CONCLUSIONS</b>Combined chemotherapy with domestic gemcitabine plus carboplatin is an effective and feasible regimen for advanced NSCLC.</p>

Objectives: The incidence of cervical carcinoma is increasing in young females.More and more clinical researches focus on the choice of therapeutic methods and improvement of the patients' survival.This study aimed to evaluate the efficacy of preoperative concurrent chemoradiation for stage Ⅰb-Ⅱb high-risk cervical carcinoma in young females.Methods: We retrospectively analyzed the clinical data of 118 female patients(≤35 years) with stage Ⅰb-Ⅱb high-risk cervical carcinoma,bulky,low-differentiation or non-squamous.According to preoperative treatments,the patients were divided into three groups,Group 1 preoperatively treated by concurrent chemoradiation,Group 2 by neoadjuvant chemotherapy and Group 3 by radiotherapy(intracavitary irradiation),all followed by radical hysterectomy.And those with pathologically reported pelvic lymph node metastasis,deep cervical stromal invasion,vascular tumor thrombosis,or surgical margin positivity received postoperative chemotherapy and/or radiotherapy.Results: The disappearance rate of cancer cells and the rate of superficial myometrial invasion were significantly higher in Group 1 than in the other two groups(P
